A Power of Attorney for Personal Care (POAH) does two things: it appoints someone to make healthcare decisions for you, AND it documents your wishes. This Living Will Wishes document focuses only on the wishes portion. It's ideal if you already have a POAH that names an attorney but lacks detailed instructions, or if you don't have someone to appoint.
