Who do you want to make personal care decisions on your behalf? ? What is an "Attorney"? An "attorney" in this document means the person you're giving decision-making power to - not a lawyer. This person will make decisions about your health care, nutrition, shelter, clothing, hygiene, and safety if you become mentally incapable.

Your attorney may become responsible for profoundly important decisions about your well-being and quality of life. If you decide to appoint an attorney for personal care, it is important that you do so of your own free will, without pressure from anyone else.
